Must be proficient in written and spoken English DUTIES:
The Adult Psychiatric Mental Health Nurse Practitioner or Adult Psychiatric Clinical Nurse Specialist (APRN) is a member of the Inpatient Psychiatry Unit (IPU) interdisciplinary t! reatment team at the Salt Lake City Veterans Affairs Medical Center (VAMC). The APRN is responsible for initial assessment, treatment, evaluation and disposition of veterans in crisis diagnosed with an acute and/or chronic mental illness. The APRN is responsible for responding to patient crises in the emergency department (ED), IPU and Substance Abuse Rehabilitation Recovery Treatment Program (SARRTP). The APRN provides patient, family and staff education. He/She will consult and collaborate with a multidisciplinary team. The incumbent will prescribe medications, refers patients to other services/resources or involve community programs to meet the needs of the veteran patient.

He/She will report directly to the ACNS (Associate Chief Nursing Service) Inpatient Mental Health.

ROLES/RESPONSIBILITES TO INCLUDE:
Obtains and documents relevant psychiatric and medical history. Formulates the appropriate differential diagnosis based on the history, psyc! hiatric examination and clinical findings.

Establishes a plan ! of care to achieve the identified mental health goals.

Prescribes medications which may include controlled substances per licensure.

Orders medications, labs, consults, and utilizes evidence based screening tools to complete assessment, evaluation, intervention, and treatment for veterans in acute crisis.

Assesses veteran for Suicidal/Homicidal ideations. Completes appropriate risk assessment and safety plan when indicated.

Collaborates with other health care providers in assessment, triage, admission, treatment and prescription of medications

Maintains open, timely communication with nursing staff of IPU and SARRTP.

Utilizes motivational interviewing, screening, and assessment tools.

Responds to ED and IPU/SARRTP for evaluation of veterans' crises and assist in treatment planning and disposition.

Facilitates admissions both voluntary and involuntary to the Inpatient Psychiatric Unit (IPU) and/or collab! orates for admission to community resources.

Makes appropriate referrals, as indicated, to reporting agencies such as Department of Family Services, Adult Protective Services, police, etc.

Completes all required documentation according to VA policies and Standard Operating Procedure (SOP) in a timely manner

Completes all relevant Clinical Reminders in computerized patient record.

Notifies current outpatient Mental Health providers, therapists and/or case managers by listing them as additional signers of computerized note.

Familiarizes self with Seclusion/Restraint policy, assesses and reassesses patients needing seclusion or restraints and documents care in CPRS (Computerized Patient Record System) according to policy.

Provides coverage for other APRNs as needed.

Work Schedule: Full time, rotating shifts (at this time the need is for night shift coverage) may include weekends and holidays. QUALIFICATIONS ! REQUIRED:

BASIC REQUIREMENTS:
U.S. citizenship. Non! -citizens may be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.

Full unrestricted Licensure, Certification, or Registration required

Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The National League for Nursing Accrediting Commission (NLNAC) or The Commission on Collegiate Nursing Education (CCNE). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing.

The completion of course work equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Studen! ts should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6)

Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia.

EDUCATION/EXPERIENCE REQUIREMENTS:
National certification in Adult Psychiatric and Mental Health Nursing required

Prescriptive privileges required

N urse Practitioner:
A nurse practitioner must be licensed or otherwise recognized as a nurse practi tioner in a State, pos sess a master's degree from a program accredited by the NLNAC or CCNE, and maintain full and current certification as a nurse practitioner from the American Nurses Association or another nationally recognized certifying body . The certification must be in the specialty to which the individual! is being appoint ed or selected.

A clinic al nurse specialist must pos sess a master's degree from a program accredited by the NLNAC or CCNE and must maintain full and current certification as a clin ica l nurse specialist from the American Nurses Association or another nationally recognized certifying body . GRADE DETERMINATIONS:
Experience as a RN will be evaluated to determine if the experience is of an acceptable level of quality with regard to the following four dimensions of nursing: Practice, Professional Development, Collaboration and Scientific Inquiry. Each dimension of practice has criteria that demonstrate essential core competencies for VHA Nursing Personnel.

Professional Development: performance, education/career development;

Coll! aboration; collaboration, collegiality;

Scientific Inquiry: quality of care, research.

Physical Requirements:
The incumbent must be able to perform all activities of the position without restriction. This position involves considerable standing, walking, stooping, bending, reaching, lifting and turning. It requires repositioning patients and assisting with transferring patients to wheelchairs/stretchers. The position requires visual and auditory acuity at a level to provide safe and effective nursing care. The position requires the incumbent to execute concepts taught in the Prevention and Management of Disruptive Behavior (PMDB) course. ability to physically control or protect him/herself from physically abusive patients.
