This position coordinates services as directed by attending or referring physician or clinical case manager. The incumbent oversees delivery of individualized services to patients and families.

Responsibilities:
  • Functions as the patients’ advocate to facilitate effective communication and appropriate treatment.
  • Orients patients to the Health Sciences Center program.
  • Works with health care team, including the patient in development and implementation of the care plan.
  • Advises patients and families regarding financial matters; assists social workers and case managers in accessing financial an! d community resources.
  • Assists in establishing and maintaining good relationships with referring physicians, third party payers and community agencies.
  • Facilitates involvement of appropriate treatment team members, timely completion of evaluations and reports and scheduling of team conferences.
  • Maintains contact with patients’ family in order to facilitate family involvement and apprise the family of patient progress.
  • Contacts payer’s and submits reports regarding the patient\\\\'s initial assessment, expected outcome, expected cost and recommended discharge plan.
  • Ensures the payer is represented in staffing conferences when requested.
Knowledge / Skills / Abilities

  • Ability to perform the essential functions of the job as outlined above.
  • Demonstrated organizational, human relation and effective communication skills.
  • Ability to provide care appropriate to the population served.
  • This position is responsible for facilitating open and accurate ! communication among patients, their families, referral providers, payers and the health care team.
  • Difficulties are encountered when patients or family members do not understand or refuse to follow the health care team’s recommendations. Receiving timely requests from members of the health care team is crucial to the incumbents’ ability to make the necessary arrangements on behalf of the patient.
  • The staff member must be able to demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patients served on his or her assigned unit.
  • The individual must demonstrate knowledge of the principles of life span growth and development and the ability to assess data regarding the patient's status and provide care as described in the department's policies and procedures manual.
Qualifications:
Required

  • Bachelor’s degree in a clinical area, health care administration, related field! or equivalency.
  • Two years experience in case management activities with third party payers.
Qualifications (Preferred):
Preferred

  • Current license to practice in a clinical field may be preferred.
  • Desire to work with and provide care coordination support to children and adults with developmental disabilities.
  • Able to work collaboratively in a multi-disciplinary team with psychiatry, counseling, and primary care services.
