Nurse Case Manager
Location: Ogden, Utah
Description: Intermountain Healthcare is presently looking of Nurse Case Manager right now, this job will be ordered in Utah. The Nurse Case Manager works collaboratively with physicians and other members of the health care team to improve the health of patients with chronic conditions and/or complex needs. Educates patients and families to help them manage their health care needs. Facilitates communication, coordinate services, address barriers, and promote optimal allocation of resources while balancing clinical quality and cost management.

Minimum Requirements
Three years RN experience.
Current Utah State RN License.
Basic Life Support (BLS).
Bachelor’s degree in nursing. Education must be obtained through an accredited institution. Education is verified.
Current Utah Drivers License in good standing and reliable and insured transportation.
Must function with a high degree of autonomy, communication and interpersonal skill.
Must understand the health care continuum and have the ability to solve complex problems.
Basic computer skills and knowledge of word processing software.
Physical Requirements
Seeing, Hearing, Speaking, Listening, Manual Dexterity
Preferred Qualifications
Ambulatory or care management experience preferred.
Work experience in Psychiatry preferred.
